Patalliro! takes place in Marinera, a tropical country in the South Seas, known for its eternal spring and significant diamond production. The country's diamond-rich mines are a key focus of the story, as they attract the attention of the International Diamond Syndicate, a powerful organization seeking to control the global diamond supply. Patarillo, the vertically challenged but wealthy king of Marinera, is protected by Major Jack Bancoran, an English MI6 agent known as the 'Bishounen-Killer', who has a unique reputation for being irresistible to adolescent males, including Patarillo.
